How to Apply For A Passport

1) Down load the form here and fill it out. DO NOT SIGN IT.

2) Bring the form to the District Clerks Office along with your original birth certificate and driver's licence or ID.

3) You will also need two photos. We can take them here or you can have them done elsewhere.

4) You will also need a check or money order for $110.00 made out to the U.S. Department of State

5) Charges from the District Clerk's office are:

$25.00 processing fee
$10.00 for the two pictures.
